Death may occur when promethazine is combined with other depressants, such as opioids, benzodiazepines, barbiturates, thienodiazepines or other GABAergic substances like alcohol.[1]

Additionally, promethazine is an anticholinergic, and at high doses it may cause delirium and extremely unpleasant if not dangerous experiences. Please be extremely careful when trying this pharmaceutical and use responsible use practices such as always having a tripsitter when using promethazine, especially at high doses.

Fatal overdose may occur when benzodiazepines are combined with other depressants such as opiates, barbiturates, gabapentinoids, thienodiazepines, alcohol or other GABAergic substances.[1]

Serotonin syndrome and/or a prolonged heart QT interval can occur with using SSRIs (such as citalopram, paroxetine, or sertraline) with SNRIs, SRAs (such as MDMA), DXM, serotonergic stimulants (such as cocaine), MAOIs, and RIMAs.

It is strongly discouraged to consume moderate to heavy dosages of these substances together.

Promethazine is a muscle relaxant and antihistamine used as a pharmaceutical. It can form interesting or mildly novel combinations with other pharmaceuticals and psychoactive substances. This guide will explore some combinations of promethazine


Prometh (NO CODEINE) with DXM ("Promethazine DM")

Promethazine and DXM forms a sort of novel state of dissociation and mild delirium.

Doses for prometh/DXM are:

Light: 25 mg and 120 mg A light dose should be started.
Medium 31.25 mg and 200 mg This dose will alter the user to potentially dangerous levels of suggestibility, delirium, dissociation, or confusion.
Strong 50 mg and 200 mg + This dose can cause fatal levels of muscle relaxation, leading to respiratory arrest and then death. It is recommended to never use a strong dose for a first try.

MAKE SURE THE ONLY INGREDIENT IN YOUR DXM IS DEXTROMETHORPHAN HYDROBROMIDE. Additives such as paracetamol, chlorpheniramine maleate, phenylephrine, and diphenhydramine or doxylamine can cause effects ranging from a severely unpleasant trip, like the last two, and the rest, to a painful, long death or hospitalization. Guaifenesin can cause nausea. Dextromethorphan polistirex will ruin the trip by lasting too long (it's basically extended-release).

Prometh (NO CODEINE) and Valium (diazepam)

This combination leads to increased muscle relaxation, which can lead to a pleasant "arms feeling like warm gel" feeling. It can be potentially fatal, however, seeing as both easily depress the nervous system.

Adjust for personal tolerance and based on other sources
Light: 12.5 mg and 5 mg A light dose should be started.
Medium: 25 mg and 7 mg This dose will alter the user to potentially dangerous levels of suggestibility, delirium, or confusion.
(very) Strong 37.5 mg and 10 mg This dose can cause fatal levels of muscle relaxation, leading to respiratory arrest and then death. It is recommended to never use a strong dose for a first try. The muscle relaxation and sedation risk is especially concerning with this combination, and therefore it is recommended to never a strong dose with this combination and no tolerance.

Prometh (NO CODEINE), an SSRI, and cannabis

This combination increases happy feelings, muscle relaxation, and sedation and mitigates paranoia and anxiety from cannabis.
Serotonin syndrome can be caused by SSRIs with other drugs.
The SSRI value is for citalopram. Citalopram can be substituted, but I found it works best for avoiding anxiety. A low and light dose of a benzodiazepine can be used, but this will add to drowsiness. I REALLY love Ativan, so I'll use that benzo. You can use other light doses of benzos. Keep in mind; light.

Light: 25 mg, 10 mg, depends on tolerance A light dose should be started.
Moderate: 37.5 mg, 10 mg, depends on tolerance This dose will alter the user to potentially dangerous levels of suggestibility, delirium, or confusion.
Heavy: 50 mg, 10 mg, depends on tolerance Promethazine at this dose can be very sedating. it is recommended to build a tolerance.
Strong: 75 mg, 10 mg, depends on tolerance This dose can cause fatal levels of muscle relaxation, leading to respiratory arrest and then death. It is recommended to never use a heavy or strong dose for a first try.

With lorazepam (Ativan)
Light: 25 mg, 0.5 mg, depends on tolerance A light dose should be started.
Moderate: 37.5 mg, 0.5 mg, depends on tolerance This dose will alter the user to potentially dangerous levels of suggestibility, delirium, or confusion.
Heavy: 50 mg, 0.5 mg, depends on tolerance Promethazine at this dose can be very sedating. it is recommended to build a tolerance.
Strong: 62.5 mg, 0.5 mg, depends on tolerance This dose can cause fatal levels of muscle relaxation, leading to respiratory arrest and then death. It is recommended to never use a strong dose for a first try.
