Death may occur when promethazine is combined with other depressants, such as opioids, benzodiazepines, barbiturates, thienodiazepines or other GABAergic substances like alcohol.[1]

Additionally, promethazine is an anticholinergic, and at high doses it may cause delirium and extremely unpleasant if not dangerous experiences. Please be extremely careful when trying this pharmaceutical and use responsible use practices such as always having a tripsitter when using promethazine, especially at high doses.

Fatal overdose may occur when opiates are combined with other depressants such as benzodiazepines, barbiturates, gabapentinoids, thienodiazepines, alcohol or other GABAergic substances.[1]

It is strongly discouraged to combine these substances, particularly in common to heavy doses.

Lean (also known as Sizzurp, Purple drank, and many other names)is a psychoactive drink containing promethazine syrup or liquid and an opiate, such as codeine or the opioid hydrocodone. Traditional lean consists of the first preparation. This guide contains ways to make both preparations.

Preparing and consuming it

Obtain a bottle of Prometh w/Codeine, or any other syrup that contains only promethazine and codeine as the active ingredient. Use the dosing cup provided with the product to measure your syrup. Usually, the syrup contains 6.25 mg of promethazine and 10 mg of codeine per 5 mililiters of syrup. It should say on the bottle what the dose is. Get a verified 5 mL measuring spoon, and, no, a kitchen spoon can absolutely not be used. Measure roughly based on the chart below.

Dosage for prometh w/codeine

Adjust for personal tolerance and based on other sources
Light: 25 mg and 20 mg A light dose should be started.
Medium: 50 mg and 40 mg This dose will alter the user to potentially dangerous levels of suggestibility, delirium, or confusion.
Strong: 75 mg and 60 mg This dose can cause fatal levels of muscle relaxation, leading to respiratory arrest and then death. It is recommended to never use a strong dose for a first try.

Dosage for hydrocodone and prometh gel cap, liquid, or tablets

Adjust for personal tolerance and based on other sources
Light: 15 mg and 25 mg A light dose should be started.
Medium: 25 mg and 37.5 mg This dose will alter the user to potentially dangerous levels of suggestibility, delirium, or confusion.
Strong: 30 - 35 mg and 50 mg This dose can cause fatal levels of muscle relaxation, leading to respiratory arrest and then death. It is recommended to never use a strong dose for a first try.

Preparing the hydrocodone

To prepare hydrocodone, paracetamol must be removed from the pills if it is in excess of or at 4 grams.

Materials needed:
A standard piece of paper
Your desired dose of hydrocodone / paracetamol (the paracetamol will be filtered out later)
100 mL of water
A glass for filtering and another for drinking
A freezer
A papertowel or clean, thin tshirt fabric
Something to crush with


Procedure:

1. Take the hydrocodone tablets. Fold the piece of paper in half. This step and the next are only necessary if your session is going to use above 4,000 mg (four grams) of paracetamol within 24 hours. You can make sure that the pills are hydrocodone/paracetamol by following this link (works for pills made in the US): https://www.drugs.com/pill_identification.html

2. Put the hydrocodones into the paper and crush them very finely. A hammer, meat tenderizer, or a papertowel holder, etc. can be used to crush and a CD or credit card can be used to fine the powder out as it's between the paper. When it is sufficiently fine, put it into your glass of gently warmed 100 mL (precisely measured) water. Stir until all of it is dissolved.

3. When it is all dissolved, put it into the freezer and check on it every five minutes. When you begin to see a large white pool of sludge at the bottom, you can take it out after putting it in for one or two more times. The white sludge is the paracetamol and pill binders. The clear solution is the hydrocodone-mixed water. You can extract it with a turkey baster. The taste of it should be like dextromethorphan's, and therefore it's good to chase it with a fruity drink or citrus soda to avoid nausea or bitterness.

4. If you want, and you probably should, filter it through a wet papertowel at least once.

Consuming it

Add your filtered hydrocodone water or your prometh/codeine to the drinking glass. If you have prometh tablets or gelcaps, simply set them aside and take them with the liquid.

Jolly ranchers
Substances (Do not add any substances that would be considered dangerous to combine with hydrocodone, promethazine, or any other ingredients)

and enjoy.

For codeine and prometh, add your properly measured dose to the glass.

Sprite or cold citrus drink (It might contain grapefruit juice, which could potentiate opioids, and could be dangerous at high doses)
